Lead Paint Disclosure Brochure on a Lease Renewal in Wisconsin

Is the Lead Paint disclosure form required to be included on a lease renewal in Wisconsin?

Not in the renewal, but it is required to be in the original lease agreement.
I always attach the disclosure JUST in case. I think its important for the landlord future protection
