Evicting a "houseguest"

I need to evict my houseguest who has overstayed her welcome and has been very passive-aggressive. An attorney advised me that I should send her a 30-day notice of lease termination and demand for possession. I cannot find a form letter that really fit the situation, and I do not want to do it wrong. There is no signed lease agreement and no rent paid.
