Tenant agreed to early move-out per my request

Due to numerous breaches of the rental agreement, I have requested that my tenant move-out early and they have given me a verbal agreement that will move out 2 months earlier than the lease agreement states. I would like to get this in writing but am unsure if there is a specific from that I use, etc. Any guidance/insight would be greatly appreciated.
