30 Day Notice to Quit Maine

Has anyone else used the 30 Day Notice toQuit form in Maine?   I did and I found out.. just when I was finally ready to go to court (since serving the notice in February...before the courts closed) and my attorney advises me that the form does not meet the statutes of Maine!    I'm wondering if anyone else has run into this issue?  Thanks!
