Rental price includes....these?

Do you always include full cost of taxes and utilities in your rental price? If not, why not?

I always have the tenants get utilities in their names and the pay the companies directly. I don't want to be responsible for a bill that varies according to the tenants actions. IE. running the air conditioner with all the windows open.
