Tenant breaking lease

If you have a tenant sign a years lease and pays on time and after four months needs to move out.. Are you required to refund her last months rent and security deposit? Place is left clean but there is a huge hole in alumunin siding which has to be repaired but she claims she has no idea how it happened..
