If the tenant gave up her dog and then went to get another without notification to us would this be considered a change in the agreement. #15 states small do is allowed per arrangements and we had the information on the specific dog under contract that she no longer has

Yes she will want to get approval of that before changing the dog. I would just notify her that you need to draw up a new pet agreement.
