Forming an LLC, transfer of contracts

I'm trying to start an LLC and I'm wondering how to amend or transfer my management contracts from my sole proprietorship to the LLC since the names are different. I am worried about accidentally voiding my contracts/leases. How do I go abo hi t doing it legally and cost effectively
