Procedure for Security Deposit - State of Ohio

New Tenant is moving into my rental property.  In the past, I've never completed the Security Deposit Receipt & Disclosure, with Bank name/address where the security deposit is being held along with the current interest rate  __ %.   I normally give the Tenant a receipt for their records.   Thoughts on if I should use this Security Deposit Notice?
