Tenant Selection Criteria

As a new landlord, i have searched forms for Tenant Selection Criteria so that I may have an idea on what I should put into it or is there a basic form that can changed to fit?

Tenant selection MUST be the same for every applicant and must adhere to the Fair Hosuing standards and Anti-Discrimination Acts.
