Authorization form

Has anyone used an authorization form to obtain verification of employment or their history with landlords?  Do we need their authorization to obtain this info?

Where do I finds this form?  I looked through the site but it must be under another name.  Any help is greatly appreciated.
Absolutely!!! You must obtain the applicants permission in order to conduct background investigations including credit.
